I place it direct into tank and add the aragamilk supplement they generally provide

I sent an email to CaribSea some time ago. You will find people that rinse it, people that don't. I didn't and have or had any issues. This was the response I received.

Hi it will cloud the water for a little maybe 1 day or so regardless but less if rinsed . I didn’t rinse the first time and regretted it because it was like 40 bags but with one or a few U should be ok no need to rinse in my opinion thx

I now always rinse new sand, also also found a few small pieces of metal with a magnet while rinsing the last time.

I am worried don’t want ick or any type of deceases, should I be worried
I don't think you have a thing to be worried about. It has got live bacteria in it and that is beneficial. I bought the Fiji Pink and the instructions said not to rinse it. The little package is to clear up the cloudiness. I have been happy with mine.

If you rinse in tap water, that would be a mistake IMO. The chlorine will kill your bacteria.
