Looks like an infection starting up, which is very common with butterflies. Three options in order of preference:

1. Start dosing NFG (if you have to mail order it, do option #2 instead): http://store.nationalfishpharm.com/NFP-products-Nitrofuracin-Green-59584.Item.html
2. Start dosing Kanaplex + Furan-2 + Metroplex (or at least Kanaplex or Furan-2)

Ok I don't have furan-2 but I have "bifuran" is that good enough? Also I won't be able to do water changes Monday morning through Thursday afternoon, is that "ok"?

Kanaplex, metroplex are in. Bifuran seems to have the same active ingredients but wants a partial water change every 24 hours during use, not sure how seriously to take that.

Lastly, do I need to worry more about WHY this happened? What could have caused that lesion in the first place? I don't expect that the flasher wrasse is responsible for this.

Here's my plan:

5/21/2017 - Day 1 - Kanaplex, metroplex
5/22/2017 - Day 2 - 25% water change redose changed water with kan/metro
5/23/2017 - Day 3 - Redose all 20g with kan/metro
5/25/2017 - Day 5 - 25% water change, redose kan/metro
5/27/2017 - Day 7 - 25% water change, redose kan/metro
5/28/2017 - Day 9 - 25% water change, redose kan/metro

If I want to add biofuran, it says to dose every day and do a water change every day. I can't do any water changes on day 3,4 and 5. Should I add the bifuran each day for 7 days anyways? Should I ignore the instructions and do bifuran every other day like the other two?

@Areseebee I would dose Kanaplex + Metroplex + Bifuran every 48 hours. Spread the dosages out over a 2-3 hour time period so you aren't dumping too many meds on him all at once.

Do a 25% water change before every other dose. So, every 96 hours.

Ok that will save me. I didn't add bifuran last night. I'll do a wc today and add it. One question though. If I do a wc today, obviously I redose the 5g I'm replacing today, but do I still dose the full 20 gallons tomorrow then (on the next scheduled dose)?

Ok I actually dosed it in the morning 8 hours later so I think I'll consider them synched. I'm going to show my wife how to do a wc so hopefully she can do one on day 5!

I have a question and maybe i'm just taking this too seriously:

When I do the WC. Do I take out 5g of water, put back in 5g untreated water then redose for the 20g. Alternatively, I could put the entire dosage for the 20g into the 5g of new water and thus the concentration never drops that much. Of course, the concentration is dropping over the entire 48 hours anyways so I'm not sure it even matters.

I'm always unsure how to picture concentration vs time with antibiotics.

You can do it either way. Antibiotics degrade rapidly when dosed into water. So, maintaining a constant therapeutic level of antibiotics is impractical for hobbyists. We are only able to do so for treating parasites, because practical copper test kits exist. Also, as this article states, "Antibiotics, in and of themselves, do not cure a fish. Antibiotics merely control the population growth of bacteria in a fish long enough for its immune system to eliminate them."

Ok, we are on Day 7 now. I did a big water change today. Here's some pictures. Looks healed over and he seems healthy, he'll eat every time except the morning after a dose. The wrasse seems a little less happy, hard to explain but he just looks a little off, but still I think he's alright.

awesome! It looks like it's still there though, so you might want to extend the treatment just a bit longer than 10 days. Since it's showing improvement anyway.
