Chalice ID

I got this chalice from a guy who got out of the hobby about a year ago. He could't remember the name or remember if it even had one so checking here to so if anyone has a clue. This thing is wicked under LED's (photo under AI Vegas just blue's at 80% with iPhone 5). The eyes are somewhat rainbow in color, but look kind of freaky against the vibrant orange flesh. I'm calling it the "Jeepers Creepers" chalice 'cause those are some wicked peepers!

Have seen chalice with that base color but not those eyes. Kinda looks like an alien cookie.
