Oh no. Tissue retraction. You can try a freshwater bath to eliminate any irritation.
Salinity and ph is important. Salinity 1.024=1.025, and ph of 8.1-8.3 and temp not to exceed 80 deg (pteferable is 76-78) and no strong current.
Any angels or butterflies in tank ? Also, with a flashlight, look into clam for presence of any crabs or tulip snails.

YES, COULD HAVE STUNG CLAM
Agree with @vetteguy53081 . It's hard to tell from the picture but I don't see any evidence of recent growth. This guy could have been in trouble for quite a while.
I do have a flame angel in the tank and had to relocate the clam about a month ago away from bta's. I did recently have to do a large water change that was slightly over 1.025 where my tank normally is. Once I get the day off I'm gonna get a plan together to get the bta's out. I have over 30 now.
