Looks like crocea

The ribs look like five distinct which is maxima and the elongation of the shell seems maxima too. Bottom may be needed. It is a bit bleached too. How large is it?

4-5 inches, also it looked like he was attached I saw his foot in the rock work but now it’s not.

Add some phyto every other day to the tank. Helps to feed them even though most say no feeding is necessary, however, there is plankton in the ocean that they filter! Won't hurt!

I have been broadcasting phyto every couple of days. The clam attached and hasn’t moved from where I put him here are some updated pics.

He's looking quite happy! Many don't agree with the phyto but at one time I had (30) clams between our (2) tanks. Back then, I dosed DT's Phyto and I had all of them ranging from 6 months to 4 1/2 yrs. Lost them all but (1) Blue Squamosa to what I call the clam plague - one each day until (29) were gone. No idea what happened! My thought is that if they're filtering plankton in the ocean, can't hurt putting some in the tank for them, pods or other filter feeders you may have.

He is a Maxima. Heathy clams should be full of tissue when they close up. Having empty air in there mean that he is loosing mass, and the shells show no new growth. This clam have all his scutes broke off, that is why someone suggested that he is a Crocea, but he is an Maxima.
He does not seem to be having any disease. Provide good light and good water for him he should do well, however he does not have the reserve of a healthy clam to tolerate any error on the reefer’s part.
Good luck.

Wow. That must have been devastating. If I had to have all clams but one die, I would pick the blue squamie as the survivor 11 times out of 10.

It was devastating! Especially not to know why! My lone survivor was the blue squamosa. He actually attached to some rockwork I had placed him on. A couple weeks later I accidentally knocked into him and sent him sailing. Guessing I damaged his bysal since he was gone a few days later.
