Clam issue

For the last several months my clam has been in a spot on a rock and has been very happy. This morning I went and looked at it and its foot (whatever attached it to the rock) had detached from the clam and the clam was upside down on the bottom. The clam was partially open and reacted whe. I touched it to pick it up. What is happening? Anything I can do?
 
It is starting to open back up. Here is a picture.

It keeps puffing up and expelling water. The big mouth will open up and then close. All other corals and inverts are fine. No new tank additions. Checking levels now. Salinity and ammonia are fine.
