This could be caused by multiple things.

- Eye infection
- Eye disease
- Nutrient deficiency
- Popeye
- Poor water quality

These are the "smaller" problems, bigger ones like cancer cause more symptoms.

They often get this from ammonia burn or use of tap water. Assure to use distilled or wwhat some stores sell called " Betta water). Ph of water important. For cure, try epsom or canning salt at 1/2 teaspoon per 5 gallons

This is freshwater betta and likely none of these issues other than water. They are born in rice patties and are labyrinth species.
 
Can you give me a more in-depth explanation as I'm changing the water today like the exact steps I take

How long have you had this fish?

The cloudy eyes (it they are cataracts) and scalloped fins can be a sign of old age. This fish just looks old to me. One thing that can happen is the that the overseas breeders sell their broodstock when they get too old to spawn. Those end up getting sold in pet stores. Bettas only live 2 to 3 years.

I'd have to look at more photos I don't quite remember how old she is
