Bought a Clown Trigger a few weeks ago and I did not quarantine it. It was fine until a day or two ago. Immediately took to my 125 gal tank, swimming around and eating.
Just cleaned and did a water change on Tuesday. Water parameters are where they are supposed to be at.
0 Ammonia
0 Nitrates
If this matters, I do have a few pieces of sharp, fake coral.
Now, the Trigger is lethargic, shakes when it does swim, hasn’t eaten and both it’s eyes are cloudy and popped out. what the heck?
 
Can you post pics of the fish? He could have an infection. Scroll down to the part of the link below about Popeye.

Have you seen any bullying?

Sometimes flukes can affect the eyes as well. Which your mentioning of the shaking leads me to believe he may be trying to dislodge flukes. If you can catch him and perform a freshwater dip you could verify one way or the other what you are dealing with and how to treat.
 
I see no signs of fluke. It only swims when I try to move it.

Flukes can be in the gills with no visual symptoms. I would highly recommend a FW dip, it sounds like he shouldn't be hard to catch.
 
+1 on flukes causing a bacterial infection with the Popeye condition result. I’d dose Prazipro with antibiotics. Need lots of O2 here during the treatment in a QT. Suggest both a powerhead aimed up at the surface and an air stone. But I’d first do the freshwater dip.
 
In my dealings with popeye, the eyes bulge, but are not milky. If both are milky Im gonna throw down that he ran into something, or was chased into something, and has damaged his eyes. Can heal by themselves, but without further attacking. If he feels under attack, then its not gonna end well.
 
Are flukes contagious? Spread from one fish to another?

Yes it is possible for the flukes to afflict any fish that shares a tank with the fish in question. With the recent addition especially.
