Clownfish disease need id

Greetings everyone,
I need help to identify the issue with my clownfish, today it lost all appetite and appears to be swimming in upper layers of tank. Additionally every now and then dashes around the tank like crazy.
Tank is 20gallon with 2 snails, LR, sand 1 zoas and 1 ephyllia.
Tank is 9 months old and parameters are all looking good,
Ammonia: 0
Nitrites: 0
Nitrates: 20
Phosphate: 0.03
Temp: 78.8
Ph: 8.1
Since the past hours it seems to be deteriorating and I'm not sure what I have to deal.

That looks like the start of Brooklynella. That is difficult to treat without a hospital tank and formalin. You could try Ruby Reef Rally Pro, but success wouldn’t be a sure thing.
Te there any other fish in the tank? If this clowns dies, you should leave the tank fishless for 60 days to ensure the disease dies out.

Looks like Brooklynella. I'm in the same boat too by quarintining my clown fishes and give them Formaldehyde 37% bath every 2 days. Change out the water the fishes were quarintine in and continue the process for about 2-3 weeks.

There are no other fish in the tank but I will attempt to move this to QT and apply Sera Costapur F, formalin requires special licence to be bought in my country.
According to Sera, Costapur contains the bellow:
Composition: malachite green oxalate 0.18 g, formaldehyde solution 5.89 g (equals 2.06 g
formaldehyde), aqua purificata ad 100 ml.

And since I already have it at hand sounds like a good bet to try it!
My DT will stay 60 days fallow as instructed!

Unfortunately Formaldehyde is controlled substance at my country and Im unable to acquire it.
