Clownfish really sick

Hey guys I was curious if anyone knew what type of disease this is and how to treat it. So she started with ich and now has these cloudy flubbery stuff on her.

How long have you had her? It could be brook, but also sorta looks like flukes (more pics would be helpful). Below is a black clownfish with flukes for comparison purposes:

I have had her almost a year. I bought a fish few weeks ago and it got ich and whipped everything out except for her and my yellow tang. Which the yellow tang is fine now and healthy
 
A pic without the flash or a light on the top would be helpful. From what I can see if what I'm seeing the while filmy stuff on the dorsal part of her body looks like might be bacterial. Can't tell with the reflection from the flash however.

^^ After seeing the latest pics, I agree. This fish will need to be QT'd and administered antibiotics. How many fish did you lose in the wipe out?

I lost five fish. All I have left is my mocha clown, yellow tang and snowflake eel. I have the mocha in the 2.5 and the others are in the 75 gallon. What type of antibiotics should I use?

I would use this combination of antibiotics: Furan-2, Seachem Kanaplex and Seachem Metroplex.

Something else to consider: It is unusual to lose 5 fish to ich; velvet (which can look like ich) would be more likely to cause that kind of wipeout. How long did it take for all 5 fish to die?

The butterfly had it for a week and half then it went away and then by the next week it was back and infected everyone else. I was treating with herbal kordon but didn't help at all.
 
Thank yall for your help but unfortunately she passed away sometime during the night.
 
Would keep a close eye on your other fish; velvet often claims the life of them all eventually. :(
