I have had this guy for four days now, and about two days ago this started. I don’t know if this is related but the night I got him, he managed to jump out of a Biocube with the lid closed, and he landed on the filter pad. There was still water going over him, but he wasn’t completely submerged. I haven’t really noticed him eating either. It kinda looked like it might be bacterial, so I started him on an antibiotic since that is the only thing I have.

Looks like Brooklynella to me. Treatment with Formalin will cure it but is not the easiest thing to dose and is also harsh on the clown so unfortunately the odds are not in his favor.
 
I'll third Brook. Do a five min freshwater dip and then get it in qt with two scoops of metroplex per ten gallons of water. I also treat with prazipro at the same time. Lots of water agitation in your qt as prazipro will deplete the water column of oxygen.
