Clownfish Spots or Disease?

Is this just strange coloration or something else? Hole in head? Black spots on each side of head. See pics, video. I got this clown 10 days ago. In QT and being treated with prazipro. Would Metro help?

I only got him 10 days ago...didn't notice it the first week, but wasn't looking for it either. Better pics are hard because he keeps moving! I'll try though.
