Clownfish velvet?

This just appeared this week my poor fish is hurting its swimming vertically and hiding in spots it normal dosent choose to go to. Tank is 3 years old fish has been in for 2 years now and no newsst additions are some zoa frags about 2-3 months ago. Will post water params soon but nothing has changed in a while is this common to just pop up like that?
 
I'm thinking what @ScottR is asking about. This patches on the head looks like mucus sheathing from Brooklynella. It doesn't look like typical velvet. White lights would help.

Since Brook is a fast killer, probably best to treat for it asap.

45min formalin dip (or 90min Ruby Reef Rally) in a separate container, then followed by Metrodonizole every 48hrs for 10-14 days in a new/clean quarantine tank.
