Concerned About Urchin

I have had this blue tuxedo urchin since 12/29/12. I bought him with a small defect spot in his tube feet/spine area. Over the past week or 2, maybe 3, it seems the surrounding tissue has started to recede.

I have been able to spot feed him mysis shrimp in the past and he ate some tonight (it is usually a 50/50 chance whether or not he will eat the mysis or just push it off). He is not a big fan of nori and I have barely any algae that grows in the tank. I do have coraline algae. So I am not 100% on what he has been eating, but moves around the tank fine and still enjoys picking up things.
Last parameters check was 3/24 (I will do tests tomorrow) and last water change was on 3/30/12 (I do bi-weekly water changes of about 15-20%).

Ammonia 0
Nitrite 0
Nitrate ~10-20
Salinity ~1.0225
I do not measure anything else right now, working on getting salifert kits to measure calc and alk.
I did add a Tiger Cowrie Snail (~4-5inches long) on 3/21 but I have not seen her going after the urchin (but I do know they possibly will). The snail is more interested in nori/herb food than meaty food. The only other inhabitants are: 2 clown fish, 1 pistol shrimp, 2 peppermint shrimp, 4 sexy shrimp, pom pom crab >150 various snails and >20 various hermits.
