Convict Tang sudden color and behavior change

So I've had a convict tang in my qt for about 4 weeks now until today no issues eats like a pig and has been a good tank mate with the other 2 fish in qt Yellow tang and neon dottyback they all play well together no aggression he was the first to come to feed and active this morning when I went to feed before work I noticed he is very very dark gray can still see stripes but very dark and was just hanging out in a pvc elbow when I tapped it with the turkey baster I use to feed he didn't swim away didn't look like he was breathing fast but definitely kind of lethargic didn't have time to snap a pic but wanted to reach out and see if I am just being paranoid or if this could be a major issue he eats nori daily mastic every other day and rods blend at night

Just got home no change ps spots are on glass not fish need to clean glass
 
are the other fish beating up on him? If so id recommend separating with crate screening or separate tanks
 
other fish leave him alone no aggression
 
I added carbon a few days ago then this started took the carbon out
 
I've had one for a couple years, it lives in my sump now as it was a real dick to all the other fish in my previous system.
 
I think the breathing is quite rapid. If it has not been chased around during the last 30 minute or so, then probably something is wrong with it. Anything potentially toxic in the tank?

Also it's using a lot of fin movement to stay balanced. Not a good sign.

I'd look at ammonia spike since it's in qt, or gill parasites to include ick or velvet
 
Carbon in part likely the issue in addition to possible low dissolved oxygen.
-What is ammonia-ph-nitrate-salinity?
-What test kits are you using?
-I suspect ammonia has increased. Any medication in the QT tank?
Stress may be the issue but do note that even though he seems pale, in general convicts have natural pale coloration which can be greenish white, yellow or grey. What will enhance colors are good water quality and diet.
Diet:
Spirulina brine shrimp
Mysis shrimp
Small plankton
Formula 2 flake and frozen
Hikari veggie diet
Hikari marine cuisine
Nori seaweed basted with garlic extract

My first thought in viewing the video is 100% flukes. However, I then noticed that some of the splotches I was seeing were water spots on the glass (grin). Still, it could be flukes, or water quality issues or velvet.

What have you treated the tank with so far? Holding fish in a QT without a comprehensive treatment plan is always a mistake...QTs are fairly marginal environments, and often bring about disease since the fish is new, there can be water quality issues, etc. You need to balance that with an effective treatment plan.

A FW dip to screen for flukes would be a start, followed by an immediate copper treatment.

he passed last night, upon inspection did a fresh water dip nothing came off added copper power this morning after 25% change at 2.25 yellow still looks ok and dottyback ok I had ruled out flukes just because I just got done with 2nd round of prazi pro
