His back fin looks chewed up a bit. Started having trouble with balance out if nowhere. You think he was attacked or stung?

Looks like possible aggression. Often they are annoyed for some reason by tangs.
What other occupants do you have in tank ?
Is fish eating ?
Is breathing normal or rapidly ?
Pics under white lighting and a little closer may be more helpful

Video... he's not eating and swimming without good balance. Other fish are yellow tang, blonde naso and foxface

This fish has a bacterial infection from what seems to stem from a wound. While this fish can be delicate, they are also proven tough.
I suggest placing this fish in a quarantine setting and treating with kanaplex by Seachem.
Other option is reef safe Ruby Rally Pro But rally will take longer and be a gamble.

This is more of a herbal remedy than actual treatment. If you cant find Kanaplex, alternative would be Maracyn 2 but assure to increase aeration via air stone with the maracyn 2

Unfortunately, this is for fungus- You have bacterial. It would be better than nothing but not the remedy

I moved him to the qt but I don't think he's gonna make it :/
