Coral Beauty Anglefish HELP!!!

Hey guys!
So to start off i bought a coral beauty angelfish about 4 days ago and he has been doing great! I also have a bi color Blenny as well as a peppermint cleaner shrimp, along with my CUC.
I have a 40 gallon tank with about 25 pounds of live rock.
My problem I'm having is that my coral beauty just as of about an hour ago began to swim very weird, almost vertically with his head up towards the surface. He would then go and breathe air at the surface and would not stop. Unfortunately I had to put him in my QT tank and observe from there. He is still doing it and looks very weak. Any ideas on what is causing this problem? thanks!!!!
 
Do you have enough gas exchange going in your DT to provide plenty of oxygen? Open top, using a protein skimmer, pointing one powerhead towards the surface of the water are all good ways to accomplish this.

Another possibility is this is a deepwater CB angel and he was not properly decompressed by the collector. Can you post a video of his behavior?
 
How long has tank been set-up?.Check ammonia level. Dose it look like it might have any parisites,are its eyes cloudy?
 
I had a problem with almost every fish from my LFS. Other stores don't seem to have the problem They come in looking pretty good but don't eat. Shortly before dying they swim funny. My water quality is good. I noticed the lips look white but otherwise no white spots or anything else wrong. Maracyn 2 seems to be the cure but I have to act fast. Good luck.
