We would need more information to help. There could be many many reasons why, so to start what are your water parameters? How long has the tank been set up? What are you using for lighting? How’s the flow? Any recent changes?

Moderate light and water flow as well as occasional feedings
Recommended:
Temperature 76-79
Salinity 1.025
Ph. 8.1-8.3
Alk 8-9
Ca. 450
Mag 1300

Tank has been up for almost 2 years but I did just upgrade from a 20 to a 40 with new leds around 3 months ago

hi how old is tank,? how long have you had hammer?

I think Alk,Mag,Ca- you should be testing for. As well as the rest of the parameters. If these are off then the coral will decline quickly. I would pick up these test kits and test. It would give you insight into your tank to know if something is off. That wild be my first step.

If all those are within range- then look at lighting and flow- and lastly could also be a contaminant of some sorts.

I’m nit expert but this would be my course of action to eliminate possible causes. Hope this helps.

any recent changes/addtions?

Lighting has huge effect especially if it was not installed in acclimation mode

I do not have a ca. Alk or mag tester so I was basically wondering if those are most likely the culprits
My suggestion- take water sample to a trusted LFS and have them check for you
Also consider an ICP Test for a piece of mind
