Coral ID

I have had this coral for over a year. This thing has been struggling ever since I got it. It was in my smaller tank where it was all but dead and I moved it into my bigger tank where it is still struggling but looks a lot better. Can anyone identify what kind of coral this is so I can better care for it. Thanks
