How Long has it been Closed for?

I would Dip it in Revive and Inspect.

Could be a Number of things.

Bacterial Infection
Fungal Infection
Zoanthid Eating Nudibranchs
Zoanthid Spiders
Sundial Snails
Zoa Pox
Mites

So may things go after Zoanthids (More than any other Coral).
