Coral white spots

Over the past month I've seen significant die off from my coral pictured below. (Sorry I don't know the name). I bought it a couple years ago and grew it from a 1" frag. Everything has been going great until recently. Any ideas on what could be causing the white spots?

That’s the skeleton coming through the flesh.

You need to go back an try to figure out what changed in the system.
 
Its a favia colony, its big too, so the growth is impressive. As stated above, has anything changed? Temp, levels, inhabitants, light, routines of any kind?
 
I haven't changed much.

Recent additions
Oct - Bicolor blenny, 3 chromis, 1 peppermint shrimp
April - cor20 return pump
May - 3 zoas from jason fox

Tank has been running 2 mp40s, 2 kessil 360s, vertex skimmer, and a sump for about 5 years. The total tank volume is about 120 gallons and I do 15 gallon water changes a month. Occasionally I'll run GFO if I see cyno. I also have an apex classic running and haven't seen any major changes. My PH fluctuates between 8.3 and 8.5 between the day/night schedule.
 
I also just remembered that back in the beginning of the summer I took a dremel to one of it's edges because it was stinging my other coral. Didn't think it was related though because it's not dying off in that section. Also want to add some more pictures if it helps. I have one SPS that's been showing a skeleton and my giant leather has developed some holes.

Dec 8th
Red Sea Nitrate - 0.75 ppm
Red Sea Phosphate - 0 - 0.04 ppm (it's kind of tough to tell the difference in colors in the red sea tests)

Nov 25th
Red Sea Phosphate - 0.04

Oct 21st
API - ammonia - 0
API - nitrite - 0
API - nitrate 0 - 5 ppm
Red Sea - Nitrate - 4 ppm
Red Sea - Phosphate - 0.08 ppm

IIRC October is when I started to notice the die off. The nitrate seemed high so I cut back on feeding a bit. I also did a couple extra water changes. Also loaded up two reactors with GFO and carbon.


RODI is 0 ppm. Tested with Hanna combo pen.
 
All your numbers are actually so low I though I was reading randy F reccomend numbers.


Honestly, he first thing I’d try Is pull the gfo.
Get some food in the tank .
