Is that a torch beside an Acro.
That’s not going to work out well for the Acro.

In general, the melting happens in new tanks with unstable waters.

I don’t even add softies anymore until 8 solid weeks of stable parameter results are completed, then as soon as I see any coralline, I put in LPS. SPS at say 8-9 months as long as they remained rock solid.

There are several factors that will contribute to such activity:
Alk, temp. CA and nitrate spikes
Flow too high or too low
changes in lighting
Too much or inadequate lighting
Unreliable test kits such as API notorious for false readings
Flatworms
Red or Black Bugs
High salinity

These are of few items to check and verify as well as reliability of your test kits especially if near expiration. One is probably that the water is too clean. I've seen tanks with ultra low nutrients melt away zoas and other softies. They need water with . . .. Po4 around 0.02. And no3 below 10 in most cases.
I would look at alk and Ph as a chief concern.

Temp 77-79
ph 8.1-8.3
salinity 1.025
nitrate < .5
phos < .04
Ammonia < .03
mG 1300
Alk 8-10
CA 440

The only thing that changed is I put that sps
(hard coral, I think I’m saying that right) in. It’s the first piece of that stuff that’s been put in.

Tanks been setup for about 10 months. I just put the arco up there. It won’t stay there
