Dark Grey spots on Hippo Tang

Sorry if the photo quality is a little lacking, girlfriend took photos this morning with her cell phone.

My google-fu says it’s likely ich, but it looks more like wounds.

He’s active and eating and none of the fish in the tank are showing any significant aggression toward one another.

How long has the fish looked like this?
It doesn’t really look like ich to me.
Could be hlle, but damage like that wouldn’t happen overnight.

That's not ich. It looks in the pictures that the fish's eyes are a bit cloudy also. If true, the tang probably has Neobenedenia flukes. I also see a bit of Head and Lateral Line Erosion.

Can you post a short video? That might be clearer than the photographs.

Maybe a week since I noticed it, but it seemed to get better and worse at first...which is why I thought it was some kind of trauma.

I will see if I can get some video up.

That does look like Neobenedenia flukes. You could try Prazipro, dosed 8 days apart, 3 times with good aeration. Keep skimmers running, but don’t collect skimmate. Turn off any UV and don’t use carbon.
A better treatment would be hyposalinity at 1.012 for 30 days, but invertebrates would need to be removed.
