DC4HD not turning OFF

I'm using my old school ACjr with a DC8 and DC4HD. Everything is hooked up correctly (that I can tell) but the DC4HD doesn't seem to be listening to the controller. For example, the controller displays an outlet is OFF but the outlet is still turned ON. Has anyone experienced this?
