Dead Clown Instant Death

I just found my female clown dead between the acclimation box and glass of my tank. Just wondering if she could have caught herself in there and died, or if that was just where she ended up. I'm pretty distraught and doubting my fishkeeping so I thought I'd ask in here to see if I'm missing anything.

The clown pair were the first fish in the tank, in there for about five months and have been thriving/eating like champs. Everyone else in the tank seems happy. I've been sitting next to the tank all morning, she ate at the first feeding time, she was fine one minute and dead the next. Could a sudden panic stuck between the glass and box kill her? I don't see any signs of sickness that I can tell.

This may be a stupid question, and I think I know the answer, but I added two rock flower anemones yesterday and wanted to confirm they were not the culprit as well. Thanks for your help.

She looks pretty thin, did she eat well? Ever see any white stringy poop?

She was the biggest pig in the tank (within reason). Never had any signs of parasites, no white stringy poop, no odd behavior. I actually commented on how well the pair had been growing (we got them as very small juvies and have been enjoying watching them grow up) just this morning. Absolutely nothing was off until I found her dead.
 
Sorry for your loss. Fish have heart attacks, strokes, just like we do. :(

The more I think and the more I look, the more I think that's the case. Heartbroken but relieved that the rest of the tank is still okay.
 
I don’t see anything obviously amiss... I agree with @Big G - these things just happen sometimes:(
