Dead clowns. Help identify cause

Hoping someone can help me identify what caused this clown to pass. This is the second one that has gone in the past three days, unfortunately. I was able to get them out after my gramma began flashing and treat in QT. However, sadly both did not make it. It wasn't quick, such as noticing off behavior and dead the next day. They both began a bit of scratching. Did freshwater did and treated with Prazi over a period of two weeks per instructions. Swimming became lethargic and after that 24 hours both were dead two days apart. I didn't sit back and hope things would get better. I rushed to get medications, I treated in QT, did frantic water changes and testing. But, couldn't save them. The gramma who was the first to show signs of distress is still alive and eating well, although still flashing and my yellow watchman seem unaffected. Water parameters were 0 ammonia, 0 nitrite, 8-10 nitrate, 1.026 salinity and 78.8 degrees. Tank was up for nearly 5 months, no new additions for more than 2 months. I did have one day when i forgot to hit the fan before leaving for work and temp rose to 81.5 for a number of hours. Truly a sad day. I love this hobby and want the best for the fish. I did hours of research attempting to figure out how best to resolve the issue and sadly I either missed something or didnt do enough...
 
Looks like it's fins are pretty nipped up. Do you have any other fish in your tank besides the gramma? The 81 degree day shouldn't have been the issue. Salinity is a little on the high side, would are better if it was at 1.024-1.025 What is your PH?
 
ph. around 8 pretty consistently. Fins were fine until today. Gramma went at the clowns now and again but wasn't ever a concern. She went today and the male went two days ago.. Color changed significantly. Not sure if i can be seen in the photo but they both began to have large grey patches.
 
did you add anything new to the tank before all this started? like fish or coral or inverts? more then likely a parasite took them out if your tank was 5 months old you can rule out ammonia or nitrite spikes also it wouldnt be salinity or your temp either
 
No, nothing new added within two-three months... gramma is still flashing at the moment. But he has shown signs of distress for weeks longer than the clowns and the clowns died. He's still very much alive and eating.
