Plenty of movement, I guess I'll take the base rock out, and maybe put PVC for the fish to hide. The lfs is good with fish it's not a petco or anything just owner operated, they did say they got it in a few days ago so maybe it just never got time to relax. I just want to make sure I don't kill another with my qt setup.
 
I really don't believe it is necessary to cycle the qt tank if I plan on doing frequent water changes, am I wrong there?

Really depends on how much of a bioload you intend to put in at once and the sensitivity of the animals.
 
You're not missing anything. If the levels were zero, then it wasn't your water. I set up uncycled quarantines all the time.

Mostly this. While we can do many things to help the fish survive, sometimes they have endured too much stress or haven't eaten in too long (some of the fish even in our good stores may not of eaten for two weeks).

If your test kit showed you had .05 ammonia then I'd definitely say the no cycle did it. This time, I'd say it's simply bad luck and the fish would likely not of made it in any of our tanks.
 
What was the salinity of the water in the bag before you slowly added QT water, and how slowly is slowly?
If the difference was large and the adjustment period too small that could be the reason.

I am thinking that this had to be the problem, I had to go to work and only added water into the bag for about 15 min after letting the float for 20 min. I normally drip for 1 1/2 hours but I was using my drip tube for an anemone and rushed the six line. Thanks guys for the input, lesson learned.
 
I've always kept some bio media or filter sponge in the main tank, so when I need to set up the qt/hospital tank, I just add the seeded media to it and has never failed me.

This is the way to go IMO, using DT water is optional IMO and really doesn't do much for you in the bacteria department. With a good seeded sponge you won't notice any difference whether using new water or water from the display.
 
I have never cycled my water for the QT just used the Main Tanks water but all you need in the QT is a heater, HOB filter system and a PVC pipe no rocks and did weekly water changes with water from the main tank as well...
However when I bought a wrasse I put enough sand from the main tank in the corner of the QT so he could sleep in it not sure if the Six line sleeps in the sand or not.

No, the Pseudocheilinus genus sleeps in the rock work.
