Dead Wrasse

New tank, live sand, life rock, added bacteria to jump start cycle, 5 weeks in, small Diatom bloom in refuge and smaller bloom in tank. Nitrates spiked a very little, changed carbon filter (It was time to do it, not as anything special), did a 10% water change. Checked salinity and other numbers, everything still in acceptable ranges, almost exact where it has been most of tank life, nitrates dropped down a little, but then the next day my 12 strip wrasse barely moved and laid on the sand bottom. Next day dead. 2 clowns and hawkfish fine, inverts fine, what just happened I am clueless.

How old was the fish and when did you add it?
Have you quarantined the fish?

Any spots, wounds, discoloration on it?

Not sure how old, but added it 2 weeks ago. It was my wife and daughters favorite. No spots, discoloration or wounds, breathing didnt appear to be labored. Did not quarantine, the refugium diatom bloom is much worse than the tanks, was afraid moving him there might make situation worse. He was hanging out around the fire shrimp while stressed, and the shrimp molted right after the water change, not sure any of that stuff is related, just stuff I noticed.
