Why do you say the sea urchin does not look happy?

In the pic, the spines look a bit erratic, I was expecting more of an evenly spread array pointing outward, but maybe this is correct for that species.

The DBG has been a problem for me, can’t keep more than 6-9 months, then poof. I have fish 4-7 years old, but this one, IDK.

My only thought is they are so effective at sifting, they deplete the bed within months.

i honestly think it’s because the fish has flukes. When I bought him he never sand sifted at all. I know it’s not the sand because my sand it more fine than the one at my lfs

I personally wouldn’t worry about sand now. Mine looks like yours and had no problem. Let us know if eyes cure and continue to fatten him up.

Be prepared for secondary (or maybe be primary) bacterial infection. Just in case!

I ended up cleaning my rock and sand yesterday. I started the goby on fluke medication Monday and have not seen any improvement. The good thing is that he is eating the mysis shrimp that I spot feed him. What is primary and secondary?

Bacterial infections sometimes comes after flukes or other parasites by open sores on skin. So in this case the bacterial infection is secondary ( results comes after or from the parasite area). Sometimes they just get a bacterial infection (primary). Since on both eyes I still think it’s flukes first (primary). BUT this still could just be a bacterial infection. This is why I I suggested doing a freshwater bath earlier to possible confirm the flukes. But since you already started the prazipro just continue course you are on.

I did a freshwater bath on Monday before the treatment for 2 minutes. He went pretty crazy. Then I threw him back into qt. He was acting a little crazy for a few minutes still but eventually acted normal.

Since it is still eating, my guess is that the fluke treatment worked and the eyes should clear up on their own. However, some flukes are egg layers and praziquantel doesn't harm the eggs, so they can hatch out and reinfect the fish on down the road. You need to consider a second treatment about 8 days after the first one.

The goby was always eating when I spot fed him. He is no longer hiding in a corner but today I plan on doing a water change and adding some more medication. I feel like once he starts to sand sift & his eye gets better. I don’t think the other eye will get better. Doesn’t look like there is an eye there

The eye looks to have gotten better since it’s no longer cloudy but he doesn’t swim much at all and still now sand sifting. Does he need more time perhaps?

hmm his eyes do look a lot better! good job! looks a little fatter too which is good! He does look lethargic and boayuancy a little off. Are you still medicating? Maybe still need to stop and let him continue recovering Let's see what @Jay Hemdal says

I tried, brine last week but ran out and ended up buying mysis this time. I can try cutting it. But shouldn’t the goby start being more active and eating by himself by now?

Oh yes, it should be eating better, I'm just trying to offer advice to help it along. The eyes look better, but there is still some underying issue here.

I scrolled back really quickly but couldn't see what you are dosing the fish with other than "fluke medication" and a FW dip. What else have you tried?

I have done 2 fw dip last week. Once on Tuesday for 2 minutes and the 2nd fw dip was I believe Thursday for 3 minutes. Last week Monday I started him on prazi pro and currently still on the treatment. Half the tank has very fine sand and the other has Carib arag sand that is fairly fine I’d say. The temperature has been around 81 but I’m turning it down today to 78 maybe even lower. That’s about all I have done. The only thing I’ve seen change is that he finally has one of his eyes back. The other one seems to be completely dead.
