How old is the system. its RODI water? Are you running gfo? If its silicates the gfo will remove them.
yes you should be past the diatoms. It is possible that it is getting past the rodi filter. It has been reported here. You can put a bag of GFO over the return pump if you don't have a reactor. be careful as it will strip nutrints (po4) very quickly.
yes its possible then you are getting some silicates from the water. You could also try "Poly Pad" its a less aggressive binder.

You cannot use RO/DI?
If true, you presumably can use DI alone, since it wastes no water, although it will be more costly in terms of DI resin.
Can you take a picture? Can you describe it to be sure it is diatoms and not cyanobacteria?

You can pass the RO water over GFO or aluminum oxide phosphate binders since they will bind silicate.
