Did this guy just up and die on me?

He was fine had it in my tank since it was a frag. Few days ago it got a film on it almost like dinos just a rusty film and now it looks maybe gone

Nothing ever just "Ups and Dies" . There is always a cause for death, if that is what's happening.
I'm guessing Dinos and GHA are smothering the coral and killing it, but a better pic without the Blue Blue lights will help.

Answer these questions to get help, your tank is most likely over run by dinos and GHA thats killing the Monti.

Please give us more data to help you turn this tank around, if you bought that piece as a frag, you certainly have some skills because you have an exceptional amount of growth.

Tanks was setup 3 years ago. its a red sea reefer 350, 75 gal main 18 gal sump

2 Nero 5 power heads and 2 koralia for the corners.

I have a fuge setup that has been running for over 2 years.

Eshopps S200 skimmer

I do 15 gal water change every suday and Dose Cal and Alk

Only use RODI

Too me looks like a series nutrient problem of too high or imbalance.
4th time I asked for water perimeters without an answer.
We aren't caring for animals 1st, we are managing excellent water perimeters and conditions, everything else then comes easy.

Salt is always 1.024

If your replenishing snails and hermits and loosing fish, you are probably battling a nutrient problem.
When you tested nitrates what were they ?
Need PO4 ( Phosphate ) test too.
Even if your test are low, the amount of GHA and dinos they are absorbing at a rate beyond testing.

I am with you. I don’t test for nitrates and phosphates often.
But if something like this happens that would be the first thing I do.

How recently have you lost fish?

I lost a lawnmower blenny 3 weeks ago. He was only in the tank 2 weeks. He seemed ok used to sit right on my algae clip waiting for food. Just gone one day.

6 months ago I lost a female fairy wrass to some sort of fungus. Could not save her even tried to treat it.

Before that I lost a tailspot blenny but that was 1 year ago. Same deal just gone 1 day.

I have for over 2 years
1 clown
3 pajama cardinals
1 yellow watchman
1 fairy wrass
1 bluestreak cleaner wrass
1 scooter blenny
2 fire shrimp - 1 paired with goby
1 pistol shrimp
1 emerald crab that I thought was dead and then he just showed up
assorted hermits and snails
bunch of bubble tips they are splitting like crazy on me actually need to get some out

Double check your rodi. I made that mistake years and years ago. Hopefully you have a meter on it to just rule that out as a source or the container etc ...

This monti has all this film on it which indicates a flow issue for starters. They require medium not overly strong flow.
I believe the substance is dino. Take a turkey baster and blow the surface loose and net the stuff that comes off. See if it returns by tomorrow evening . If it does, time to go to battle
