Yep, I agree, get him and the rest of your fish into qt and treat with copper or cp asap, and hook you to leave that tank fallow (no fish) for 76 days. Hard lesson to learn, qt all new fish for 4-6 weeks.
 
Spots look too big for velvet(look halfway down the body)looks like the start of sloughing skin (Brooklynella). Let’s get another opinion. @4FordFamily @HotRocks
 
This is what I’m seeing. Any new additions? How long have you had him? Do you QT?

Whichever way this goes velvet or brook I would strongly recommend a FW dip to get him some relief.
 
Could be velvet, but those large spots look more like clumps/aggregates of smaller spots which is seen in brook. Interesting if it is, I haven't seen a wrasse with brook.
 
A freshwater dip, followed by an acriflavine bath (90-minutes, in a separate container, aerated and temp-controlled)

Then placed in QT pre-dosed with metronidazole. And begin ramping up copper.

Will treat both... but with either culprit time is not on your side so I wouldn’t waste time if you are going to give it a shot!
 
If you are able to get pictures under white light might make it a little easier to narrow it down ;)
 
I agree with the above suggested advice by @ngoodermuth.

There is definitely ich present. Possibly flukes and velvet as well. Best course of action is as she suggested.

If you can get some pics under full whites we can probably narrow it down.
 
I agree you are dealing with likely velvet, ich, and perhaps even brook. Velvet without a doubt, to me. Metro and copper is my prescription as well!
