What do you guys think?
I don't remember any spots above the eye at the LFS, but I can't be 100%...

Does it look like anything? Tang is eating algae pellets well and has been in QT for 2 weeks now.

Just observation QT? Or medicated?

Humblefish's treatment advisory for HLLE:

Head & Lateral Line Erosion "HLLE":

Symptoms - HLLE literally looks as though the skin is rotting or peeling off the fish. Affected areas usually include the face and lateral line, and may be white in color.

Treatment options - This is more of a condition than an actual disease. Since no one can agree on the exact cause, all you can do is address all of the following theories regarding HLLE:

1) Stray voltage - Use a titanium ground probe & test your tank for stray voltage.
2) Running carbon - Don’t use it, or use only premium grade carbon.
3) Nutritional deficiency - Feed high quality nutritional foods. Some have had success reversing the effects of HLLE by soaking food in “fish vitamins” and also feeding nori.

So the only thing I can do is really just feed him with some vitamins etc?

Water quality in QT is showing 20ppm nitrates, should I do a water change as well, or try to keep everything as stable as possible?
Any ETA on how long I should keep in QT before its safe assuming the Tang does not go downhill fast?

High quality foods, like prepared frozen adding vitamins. High quality Nori. You can also soak the nori in supplements if you want. HLLE will usually heal itself given healthy diet and pristine water conditions.

As far as, observational period, that's up to you. Whatever you normally do should be fine. HLLE shouldn't change your normal protocol and it's safe to put him in DT population. It will not impact the others.

Oh ok so it’s not contagious then. I will move him over at 5/6 weeks if no sign of ich or other infections.

He seems to like algae pellets and wafers, so I’ll get some more Zoe vitamin supplement today and soak it in that before I drop in the tank.

Correct it's not contagious.

Check into Zoecon. It's by Kent marine. I think they make a product called Zoe, and Zoecon. I have heard that Zoecon is best suited for HLLE.
