Do you change ceramic media?

I've read that ceramic filter media gets clogged and has to be changed once in a while. So I bought some new media. Then I read on the box of that brand that it never needs to be replaced. (These are the little 1/2" cylindera.)

Who changes there ceramic media? How often? How do you know when it needs to be changed? If it's just based I time, how so you keep track of when each bag was changed (since you probably don't want to change it all out at once).

I would imagine if you are using it in a cannister, then it would become clogged over time, though rinsing rather than replacing seems like the better remedy. If you are using it in a bag in the sump, then cannot see why it would clog.
