puncture from an urchin or anemone sting which both should heal on its own but do keep an eye on area

This would possibly explain mine as I have around 8 or so tuxedo urchins in my tank...although their spines are small. The other culprit could be my bristletooth tang, but those generally are not round lesions.
 
Doesn’t look like a flatworm but because fox faces lay on the bottom or up against glass when resting or scared, flatworms will sometimes crawl onto them
