Been using FlatwormStop for 3 months. Worked very well but now seeing flatworms again. The FlatwormStop liquid has turned brown from original clear. Does it lose its efficacy over time?

Everything losses efficacy over time and by being exposed to the environment. I don't know of any studies done on any of our reef treatments/dips so who knows how long it lasts.

Is it possible you contaminated the bottle? I would say that is more likely after only 3 months of use.

If it was actually going bad in 3 months I would hope the manufacturer would call it out as such.
