Not sure if this is PM or not. It has looked like this for the past 2 days and a few hours today. Thanks in advance for the help!

Hmm, kind of but I am not convinced yet.

What are your params, Cal Alk and Mag?

What fish and inverts do you have in the tank?

What supplements do you add and how do you add them?

Any other changes to the tank recently?
 
Cal: 440
Alk: 9
Mag: 1350

Fish are 2 true percula clownfish, tail spot blenny, yasha goby

I add Bionic 2 part every other day

Nothing has changed recently but this is a new clam. I have had this one for less than a month now.
 
I am wondering if the Blenny is nipping it. Can you make a shield out of a clear plastic bottle cut in half and fill it with 1/4" holes for water circulation. Then place the bottle over the clam for a few days?
 
Also, watch to see if the clowns are trying to host in it. This can tick off (mainly smaller) clams.

While lights are still on in the tank, turn off all lights in the room. Sit back and observe without moving much. Fish are like little kids and act differently when you're around ;)
 
The clam has been looking better today. I was also thinking the blenny might have nipped it but I've never seen him do it. The clowns host my frogspawn so that isn't the case.
 
As Stunreefer suggests, the offending fish usually knows when you are watching. They are indeed like children and only do it when you aren't watching.
 
PM will not go away unless you treat it with fw dip or something else. If it is looking better than it's most likely not PM. That's good news!
