I've had this Duncan for 6 months now and its been doing just fine but today it was all retracted and has a tear on its mouth is there anything I can do for it?

That doesn’t look good. A healthy duncan in good conditions should have at least 4-5 heads growing after 6 months. I suspect it has been struggling for a while under less than ideal water quality and/or lighting. It’s probably too late to save it. You could try a large water change and see if it helps.

Agreed except I think it could be saved if he figures out issue ASAP. For reference here is 30 and 60 day Duncan growth in my abused tank.

I hope you’re right, but if you look closely at the picture, it looks almost like the skeleton is hollow with just a thin strip of flesh covering a portion of the head…. If that’s the case… yeah

This is severe recession and likely due to water quality issue which caused it stress. I would verify your phosphate-nitrate-salinity and you want to feed these coral mysis shrimp. Also white intensity seems a little bright for their liking. Moderate light and moderate to medium flow
These were 2 small heads when i got them:
