So recently in the past month and a half, I have a medium sized Duncan head
(not Aussie) that has grown three heads on the side of it but its skeletal structure has not actually grown. Is this normal?

Yes, it will slowly grow out, just like how a tree branch starts as a bud.
+1, mine started as 3 heads and is now the size of a large grapefruit.

I've had this frag for about 4 months and only now has it started producing new heads. Not branching off, but just buds on the side of an existing head. None of the skeletal structure has actually grown.

That's how they do it. When those polyps can start feeding it will start growing fast.

Just make sure your parameters are in check. They all start as a small fleshy polyp on a stalk, and on a stalk of just a few heads I doubt it will branch very far out at first as it doesn't have to.

so lemme understand, they start slow and the more heads they get the more it can feed itself thus they grow faster?
