Dying Anemone

Seems like my anemone is dying, what signs could I have noticed to prevent this? Doesn’t seem like there’s any saving it now, right?

deflated for extended periods, moving around / falling off a lot, gaping mouth, loss of color
I did notice in the past couple days it was deflated, and this was right after it got moved when we cleaned the tank, I should have known this was a red flag :(

Is it definitely done for, though?

Watch it and assure it does not disintegrate. You could try Cipro Pill treatment although nem looks very weakened.
Assure low flow and moderate - not bright white intensity.
 
I recently had this issue with a BTA after it split. I put in a shady spot and feed it a chunk of raw shrimp and in 2 days it was almost back to normal.
 
Idk that kinda looks like its disentegrating, its kinda hard to kill anemone these days, there might be something wrong with the water or it got an infectio.
