Dying fish

I didn’t really know where else to post this but it seems like every single time i dump in phytoplankton to my tank i have fish die. The first time i did it i had a coral beauty angel die, i didnt put any phyto in for a while and everything was fine, the next time i added some my jewel damsel died, didnt add any for a while and everything was fine and then last night i added some and my yellow tang and damsel fish died. Is it possible this is all some big coincidence?
 
I went ahead and threw away the bottle. Does anyone know how to check if a bottle is rancid for the future? Just so i dont buy another one and lose more fish. Also thank you everyone for you help!
