I'm pretty sure my diamond watchman goby is dying. Came downstairs to find him in my 20 gallon tank that holds my water top off. He must have jumped out of my 120 and miraculously landed in that. However, when I put him back in the 120, he didn't seem to be doing too well. kept sinking tail first to the bottom and shooting back up. Now he's just kind of laying on the sand bed. I noticed a bright yellow color around his gills. Is it possible he picked up a disease from sifting the sand?

The yellow color isn't a disease - he's just been through an uncontrolled freshwater dip.

Make sure he's breathing - stimulate him, if you think it'll jog him into taking a breath, or gently hold him facing into a current.

Thanks. I took him out and put him in a bucket of water from my tank. To be honest though my water top off has some saltwater in it. Maybe not as much as my tank but I put prob 70% fresh, 30% salt in it. Thanks for the tip though.
