Came home from being gone two days and found this. I figure he’s dead since his eyes are still intact, but what do you guys think? There seems to flesh in his belly that looks like stacked lines. If he is in fact dead, what are the most common reasons of death? There’s more than enough food in my tank, and I’ve had him for about a month. Getting water parameters checked tomorrow.

My first guess was a molt until I saw what looked like flesh inside his stomach flap. Also all the molts I’ve seen haven’t had eyes, but is it possible the “eyes” were left on a molt?

Not that I can recall eyes usually are not there on a molt. Sorry to hear this.

Agree, if no movement especially when nudged is dead. A molt is generally clearer and hollow where this looks meaty yet.
Nudge it- if no movement, turn it over, then you'll know.
Assure nitrate and phos not elevated and calcium not low when you get water tested
