My storm clown suddenly has a bad eye and is laying on the sand? Can anyone diagnose? Tank parameters are within recommended levels low nitrates no ammonia or nitrites temps 80

I'm sorry my friend but with that much green hair algae growing on your sand you definitely dont have low nitrates. Not that nitrates had anything to do with your fishy though. They are simply undetectable as they are being consumed by the algae faster than your tester can indicate.

Did you already see this one using the search function though? Read the 2nd post in this thread and see if any of that can help you. It's definitely right up your alley with what your fish looks like. I hope you can help him!

I have high phosphates in this tank, nitrates have been at .4 since I set it up. There’s literally two small fish in here and I water change biweekly
 
this looks lie the result of an injury. High ammonia could also be a possibility. Give the fish a 4-5 minute freshwater dip (same temp as display tank) with a pinch of salt.
Treatment preferably in a quarantine setting will be ethromyacin ( better known as Maracyn 1)

there was one day where she was just swimming erratically and since then its been off and on the same so injury is possible but theres nothing in the tank that could bother it, still eats and no ammonia at all.

They startle easily and will crash into objects
