Euphyllia HeadS Tentacles Fall Off

Woke up to a baby euphyillia heads tentacles/sweepers floating around the tank. I’m just wondering what would cause this. Also does the head have a change of growing more tentacles out of the same hole/head?

Need to check all parameters, especially salinity. Once a head bails I’ve never seen one grow back

I would frag it to avoid more tissue loses. The coral is dying.
