Favia coral dying?

over the past couple of days I have noticed my favia coral is showing signs of tissue Lost. I can't tell if there is a pest eating it or just slowly dying. There are holes in the flesh all over with white spot all around.
Alk-8
Cal-420
Mag-1250
Po4-0.05
No3-3-4ppm
I have not changed anything or add anyone new. It would be hard to do a dipping seeing that the flesh has grown off the plug right on to the rock. Can someone help me out on what to do here. Thank you in advance.

Really? I would of never of thought about light. I have had it in the same spot for about 3 months. It was always puffed up with the feeding tentacles out. I guess then I should try to place it higher up in the the. To see if that helps
 
What kind of lights is it under? How high are your lights above the water level? How deep is it in your tank below water level?
 
Favias will also react to certain algae growing around them. I have had some die back or spotty and some not affected. Very weird. Just make sure you have light flow, good light and proper food in the water column. These guys eat. They are usually pretty hardy corals. Also, did something bump it? Any fish perching? Things like that can make them go a bit spotty for a bit. Also, could react to other corals toxins. What is in the tank?

The tank in a IM fusion 40 with a Maxspect Ethreal led. Lights are 8in from water surface. Coral is about 12 1/2 in below water surface.

For flow I have a mp10 running at 60 percent on reef crest mode. About 8in above coral running on the opposite side of tank. I have 2 clownfish and a diamond back goby in tank. With about 10 snails and 4 blue leg hermits. I have never seen any fish or inverts climbing or picking at the coral. There is some GHA growing around the tank. For other corals I have a 2 frogspawns,2 hammers, 2 torchs, 1 green Duncan, 4 different zoas. 1 green birdnest. And a mushroom. All other corals look great. The only other coral in close proximity is a mushroom everything else is away from the favia.
 
Ah GHA!!! I had about 1” of it around mine before my sailfin tang decided it was the right size for eating and it bothered my favia. With that said that very well could be it. I would clean the GHA at lest a few inches away from it and see how that goes. I still think it could use a bit more light by moving it up a few inches. Mine is about 10” deep under my AI’s at 10” above the water. Not very knowledgeable on your lights but I believe mine are a lot more powerful.
 
Those Euphyllias will have some sweepers, but you would see more damage I would guess. The mushroom may send out some slime, but not a big ordeal. Just make sure to clean the algae around it regularly. I find this helpful for the favias to grow.
 
The favites I've kept typically were not high light coral and did best in medium to lower light. They love to eat and the one we have now puts out feeders when food hits the tank. I had a similar reaction when the coral was to high in the tank. Keep a close eye on it.
 
Looking at the info on the light he has if he has it directly under the center of the light at the depth he has it at it is only receiving 100-150 par and that’s if he is running all channels at 100% I think it’s a combo of the GHA and to low of light.
 
Here’s mine that I got as a frag that only had 2 heads 2 1/2 months ago sitting at about 250 par with medium flow.
