I had the feather duster worm about 4 months from one head and it became two heads and today when I see it both heads fall off from the shell. Is this normal or something wrong with it? I don’t know what’s wrong with it.

Unfortunately when they bail from the shell it is a bad sign....some regrow a hard tube so fingers crossed!

Hang on. Leave the tube alone as at least twice i had this happenned to me and then about a month later I saw a ghost when new worms emerged !!
On the OTHER HAND though, they generally pop their feathers out because theyre hungry to catch food. When
feather dusters pop their heads off, its due to stress. Either water quality issues or an harassing tankmate can result in the featherduster popping its crown. Even at water change if you accidentally pour new water right over it.
Loosing the feather crown is generally considered a bad thing as this is the feeding aparatus for the worm. 90% of the time the reason why feather duster pop their heads is due to lack of food rather than stress. The feeding crown is too costly for the worm to lose, and the worms get really skinny because of this. So yes the worm will grow the crown back, however if you don't keep it well fed, it will pop it again and usually the worm will die after a couple such episodes.
