Fish dead do to palytoxin?

So long story short. Got 2 clowns, one beat up the other to the point of ripped fins and pop eye on one eye. Set up a 10 gallon for the little fella and started using it as a frag tank. Bought a frag because it had some purple hornets on them and fragged because I didn't want the captain jerks on it. They got a little mucusy. Didnt think much of it. Just got back from a run and went to check on the tank and the little giy who had completely healed his eye was sucked against the power head dead. Idk what else could of caused it. The rhodactis and other zoas all look terrible in there too. About to do a 75 percent water change and add carbon.

i feel terrible about this. Poor little dude made a heck of a come back and was always excited for food. Seriously words cannot deacribe how bad i feel about being so careless. Should have just tossed the ugly thinfa. Now i really hate “captain jerk palys”
 
Could be alot of things.
Completed cycle on the tank? Maybe ammonia?
Fish was already weak from being beat up? Maybe powerhead was just too much for the fish in the weak condition?
