Fish dying is happening again

Hello all. I have had a hard time keeping new fish alive in my 120g. I've taken all of the advice and (as before) the parameters are all all good.
0 Nitrite
0 Nitrate
1.024 Salinity
Ammonia 0
Phos 0
Alk and Calc are normal
78 degrees

I have a small toby puffer, 2 chromis, a banded shrimp, and a koran angel (juvenile) along with some frags. I started a QT tank and had a clown and a blue tang in there for 2+ weeks. Both were healthy, eating, and looked great. I put them in the DT (the water is the same so I didn't acclimate). The Koran was a little aggressive so I put the clown in a holding pen. The day after the tang was in there, he disappeared. Once the clown was put into general after 24 hours, he lasted 1 day and was dead in the morning.

I did the exact same thing with the Koran and he did great so I can't understand what happened (unless it was stress from said Koran?)

Any thoughts?

Fish dying and zero readings are not making sense
What test kits are you using?
also do a comparison with your test kit by taking a water sample to a trusted LFS that does not use API test kits and see what readings they come up with
Consider also sending in an ICP TEST
 
Too many variables to discern a cause at this point. As Vetteguy suggests, confirming your water test results is a good place to start. ICP would be my suggestion as well. If things come back looking good, then it might be stress from other organisms in the tank (fish, crabs, etc.) or some other predation from an unseen critter. Get a red flashlight and view the tank at night to try to spot any unknown predators. Could just be fish on fish bullying, but who knows. If your current fish like the Koran and Puffer last, I would guess it is not the water, but fish bullying or some other cause...
 
Can you get your ammonia, salinity, and temp confirmed from somone else? How big is the banded shrimp? They have been known to attack fish. Could one of the other fish that disappeared been a jumper? If none of these I would suspect something in the water whether a chemical or parasite/disease.
 
A general rule is to add the most aggressive fish last, not first. Trade in the Koran, and start over with QT for new fish, but hold them for 4 weeks, not two. Good luck
