Fish dying

Guys I need help I’ve had a problem with GHA for about a month that I have Been brushing off that’s seems to be slowly getting better however last week I lost a copper banded butterfly and now tonight a black and white hen I tested the water and added pictures please help

Can you provide us with your stocking list? The fish in that first picture looks like he got severely picked on
 
Clown , blue tang ,yellow tang , 2 green chromos and a chocolate chip star fish , a few crabs and snails
 
How old is this tank? Heniocus shows severe aggression and I would look at chromis as possible bullies, and if not clown, especially if it is a maroon or sebae clown

I also would highly suggest to take a water sample to a trusted LFS and have them test to confirm and compare readings.
Also- what is your salinity and water temp?
